 
Filing Insurance Claims
Dr. Joaquin is a participating provider for most local insurance companies, including Anthem, Harvard Pilgrim, Cigna and Medicare. If you are unsure if Dr. Joaquin is in your insurance network, or of your chiropractic benefits, please call the customer service number on the back of your insurance card.
As a courtesy to our patients, we are happy to file claims to your insurance company for services rendered. To make sure we can file these claims promptly and correctly, we ask that you bring a current insurance card for us to copy.
We cannot guarantee that by filing claims payment will be made. Individual benefits are determined by the contract between your employer and the insurance company. Please contact your insurance company or human resources department to determine your coverage. You, the patient, ultimately are responsible for payment of all services.
Motor Vehicle Accidents and Worker's Compensation
Accident claims require additional paperwork. Please allow time to fill out forms.
Motor Vehicle Accidents: Please be prepared with the name of your auto insurance company, agent's name, claim number and claim billing address. Also, it is helpful to bring copies of records from emergency rooms, police reports, and physician visits if applicable.
Worker's Compensation: If you have been injured at work, notify your supervisor immediately. It is important to follow your company's policy while seeking treatment. By state law, your choice of medical treatment depends on whether or not your carrier is using a managed care
program. If they are, you must choose a doctor within the managed care network. If
you are not subject to managed care, the choice is yours.
We are obligated to file any claims that are a result of an injury to the appropriate insurance company, whether Worker's Compensation or your Auto Insurance. Your health insurance WILL NOT pay for injuries that occur at work or as a result of an auto accident.
